WOMEN'S COACHING ALLIANCE
Assumption of Risk, Medical Authorization & Concussion Release
This acknowledgment applies to the adult who signs during registration (the “Signing Adult”) and to any minor participant registered by that adult (the “Minor”). The Signing Adult agrees to these terms both for themselves as a participant and, if applicable, as the Minor’s parent or legal guardian. References to “we,” “us,” and “our” below mean the Signing Adult and, where applicable, the Minor.
“Releasees” means Women’s Coaching Alliance (“WCA”) and its authorized sponsors, partners, partner organizations, affiliates, officers, directors, employees, volunteers, and agents.
1. Assumption of Risk & General Release
Participation in sports clinics and related programs—including drills, scrimmages, physical activity, and use of equipment—carries inherent risks of injury, including falls, collisions, sprains, fractures, concussions, and other bodily harm. I voluntarily assume these risks for my own participation and, as parent or legal guardian, acknowledge and accept these risks for the Minor’s participation.
To the fullest extent permitted by law, I, on behalf of myself and, where applicable, as parent or legal guardian on behalf of the Minor, and on behalf of our respective heirs and representatives, release, waive, indemnify, and hold harmless the Releasees from claims and liability arising from either or both of our participation, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES.
2. Medical Authorization
I authorize emergency medical care as deemed necessary for myself and, as parent or legal guardian, emergency medical treatment for the Minor when necessary to preserve life, limb, or well-being. I understand that I am responsible for medical expenses incurred for myself and, where applicable, the Minor.
3. Concussion Information & Acknowledgment
A concussion is a serious brain injury caused by a bump, blow, or jolt to the head or body. Symptoms may appear immediately or hours later and can include headache or head pressure; nausea or vomiting; dizziness or balance problems; vision issues; sensitivity to light or noise; feeling sluggish or foggy; confusion or memory problems; and irritability.
Consistent with applicable law, applicable league or program rules, and, where applicable, CIF Bylaw 313, any participant suspected of sustaining a concussion must be removed from activity immediately and may not return unless and until permitted by applicable law, WCA policies, league or program rules, and any required written medical clearance.
4. Electronic Signature & Acknowledgment
By electronically signing this waiver as part of registration, I confirm that I am at least 18 years old; that I have read, understand, and voluntarily agree to all terms above; and that I acknowledge receiving the concussion information. I agree for myself as a participating adult and, if I register a Minor, I confirm I am that Minor’s parent or legal guardian authorized to agree on the Minor’s behalf. My electronic signature applies to both of us, where applicable. This release is governed by the laws of the State of California.
